How to Register with the Practice
Our preferred method of registering new patients is by using the documents available below, which can be printed at home, completed and then handed in to the surgery.
We accept patients resident in Neilston and Uplawmoor without exception.
Our practice staff can confirm if your home falls within our catchment area of post codes G78 3– and G78 4–.
The practice operates a non discriminatory policy in accordance with NHS regulations.
Once you have completed the entire document for the required registration type, (adult or child), then hand it in to the surgery.
Please ensure that you have completed all of the form before you hand it in to reception, or we will be unable to register you as a patient with the practice.
Please note that it can take up to 4 weeks for your medical records to be sent to us by your previous GP (if applicable).
